We performed Failure Mode and Effects Analysis on subsea pipeline inspection system, identifying 47 potential failure modes and implementing risk mitigation strategies.

Benefit:
Prevented three critical failures during operation, avoiding $1.8M in emergency repairs and 15 days of production shutdown.
